Typical absence seizures of childhood absence epilepsy (4)

This video EEG was performed to assess the effect of withdrawal of medication in this 11-year-old girl. Neither she nor her parents were aware of clinically evident absence seizures. However, brief absences were documented during hyperventilation and intermittent photic stimulation. Note that absences are brief and inconspicuous. Impairment of consciousness is documented only with breath counting. She makes no error in counting, but there is a significant gap between the breath count prior and after the generalized spike-wave discharge (GSWD). Intermittent photic stimulations induced 3Hz GSWD with some polyspikes at their onset immediately after eye closure. Note eyelid fluttering at onset of the GSWD, which did not occur in spontaneous or hyperventilation-induced absence seizures. (Contributed by Dr. C P Panayiotopoulos).
