Fomepizole is a competitive inhibitor of alcohol dehydrogenase. In methanol poisoning, administration of intravenous fomepizole blocks the conversion of methanol to its toxic metabolites (formaldehyde and formic acid), allowing slow elimination by the lungs and kidneys in combination with a tolerably safe rate of formic acid formation in the liver.
