Hypoparathyroidism is an uncommon endocrine deficiency disease resulting from decreased function of the parathyroid glands, with underproduction of parathyroid hormone. It typically presents with manifestations of hypocalcemia because this interferes with normal muscle contraction and nerve conduction. Affected individuals can experience perioral and acral paresthesias, twitching in the facial muscles, muscle cramps, and tetany, but many also report fatigue, chronic headaches, insomnia, bone pain, and crampy abdominal pain.

Neurologic conditions associated with patent foramen ovale include stroke, particularly cryptogenic stroke, and migraine headache, although direct causality has not been established. PFO occurs in about 25% of the population and is asymptomatic in most people.

Primary exercise headache is a rare headache syndrome in which headache can be induced during or after physical exertion. Phenotypically, the headache begins within 30 minutes of cessation of exertion, typically lasts up to 4 hours, and can be associated with migrainous features. Intracranial pathology can present with exercise-induced headaches, and, therefore, it is imperative to investigate for secondary causes.

Ankylosing spondylitis is a progressive inflammatory disorder of the joints that may also affect extraarticular tissues. The hallmark initial symptoms of inflammatory low back and joint stiffness usually first appear insidiously in late adolescence or early adulthood. Reminiscent of multiple sclerosis, ankylosing spondylitis exhibits a wide range of clinical heterogeneity, including the occurrence of spontaneous remissions and relapses.
