Until recently, the only FDA-approved treatment for acute ischemic stroke was intravenous rtPA given within 4.5 hours of stroke onset. New mechanical thrombectomy techniques have greatly improved recanalization rates for patients with acute ischemic strokes caused by intracranial large vessel occlusion. Multiple randomized, controlled trials have demonstrated the efficacy and superiority of mechanical thrombectomy over standard medical care in carefully selected acute ischemic stroke patients.

Headache associated with neurologic deficits and cerebrospinal fluid lymphocytosis (HaNDL) is a self-limiting syndrome that presents as a new, migraine-like transient headache with temporary neurologic deficit and CSF lymphocytosis. Neurologic symptoms can include sensory deficits, aphasia, motor deficits, and visual disturbances. It is a frequent mimicker of acute stroke. Symptoms typically resolve within 3 weeks but can last up to 3 months.
