Stiff-person syndrome with vitiligo

This woman with autoimmune vitiligo exhibits severe stiff-person syndrome. The examiner is unable to bend her knees. The patient is unable to stand unassisted or take a step. She partially improves after spinal anesthesia. (Contributed by Dr. Joseph Jankovic.)
