Recurrent painful ophthalmoplegic migraine with sixth nerve palsy in a child

A 10-year-old girl had infrequent attacks of recurrent painful ophthalmoplegia affecting the right sixth nerve. She also experienced migraine headaches without ophthalmoparesis, and her mother had migraines without aura. There is a right lateral rectus palsy and enhancement of the prepontine right abducens nerve during an attack, which resolved when symptoms remitted. (Contributed by Vivek Lal MD.)
