Bilateral gluteal necrosis in pelvis (CT)

A 22-year-old female spent 20 hours in an immobile recumbent state and was found to have bilateral leg weakness. Electromyography localized the lesions to be affecting the sciatic nerves bilaterally, and subsequent computerized tomography of the pelvis demonstrated marked hypodensity with interspersed hyperdense regions indicating hemorrhage within the gluteal muscles bilaterally, with the areas of necrosis surrounded by solid arrows bilaterally.
