(A) H&E stained section of a CNS germinoma showing poorly differentiated tumor cells with uniform, large round nuclei with vesicular chromatin and single prominent nucleoli. Scattered groups of T-lymphocytes are present. (B) The large neoplastic cells and the smaller lymphocytes can be readily distinguished in smear preparations. (C) Immunohistochemistry for placental alkaline phosphatase is a marker used in the diagnostic workup of suspected germinomas. (D) Immunohistochemistry for OCT3/4, a member of the POU class of homeobox genes, has emerged as a robust nuclear marker for CNS germinomas. (Contributed by Dr. Santosh Kesari.)
