Atrial myxoma is the most common intracardiac tumor in adults, and can manifest with embolization (which may lead to stroke), cardiac and obstructive symptoms, and systemic manifestations including paraneoplastic syndromes. Transient ischemic attack is the most common initial neurologic manifestation, with strokes that often recur. Brain infarcts can also accrue asymptomatically, resulting in multi-infarct dementia, or they may present with a single massive embolic potentially fatal stroke or with rapid multi-organ failure and death due to embolic showers.
