Extrapyramidal side effects are seen with all first-generation, and most second-generation, antipsychotic drugs. Acute akathisia and acute dystonic reactions are usually the first extrapyramidal side effects to develop, primarily shortly after initiation of an antipsychotic or a dose increase, and generally before parkinsonism develops. Acute dystonic reactions consistently resolve when the offending drug is stopped, but akathisia has a more complex prognosis.

Limb dystonia consists of sustained, repetitive, and patterned contractions of muscles producing an abnormal posture of the upper or lower limb that may be present at rest, when changing position, or when performing a specific motor activity. Limb dystonia may be the initial presenting sign in neurodegenerative disorders like Parkinson disease, progressive supranuclear palsy, and corticobasal ganglionic degeneration.

Toxic peripheral neuropathy is most commonly encountered as an adverse effect of medications, but environmental and occupational exposure can also result in toxic neuropathy. There is generally a temporal and dose-response relationship between symptoms and exposure and consistency of disease in those with similar exposures. Improvement (or at least stabilization) of neuropathic symptoms should occur when exposure ceases.

Pure autonomic failure is defined as an idiopathic sporadic disorder characterized by orthostatic hypotension, usually with evidence of more widespread autonomic failure and no other neurologic features. It remains a diagnosis of exclusion. Specific attention should be directed toward some of the clinical features that were found to predict future development of multiple system atrophy or dementia with Lewy bodies in cases that are initially diagnosed with pure autonomic failure.
