Under normal conditions, Luxol fast blue stains myelinated axons blue. In this patient with CADASIL, there is a large patch of subcortical white matter (outlined with arrows), which fails to stain blue, reflecting ischemia-induced axonal and myelin loss. The cerebral cortex (CC) is stained pink and appears healthy. (Contributed by Dr. Steve Moore and Dr. Jill Hagenkord of the University of Iowa Department of Pathology.)
