This 76-year-old man presented with diplopia in all directions of gaze. The right periorbital tissues showed diffuse boggy edema concentrated in the inferior lid. There was significant clear chemosis of the right bulbar conjunctiva. The globe was relatively immobile and displaced superiorly by several millimeters. The differential diagnosis included unilateral thyroid-related orbitopathy, lymphoma, and vasculitis. (Contributed by Dr. Mark Duffy.)
