Sclerosing orbital pseudotumor (CT)

A 54-year-old female presented with a history of progressive proptosis, ptosis, diplopia, and periocular pain over a 6-week interval, despite prednisone treatment. CT scan revealed a mass that indented the globe. Orbital biopsy revealed a dense fibrotic mass consistent with sclerosing orbital pseudotumor. A debulking procedure and release of the superior and medial rectus muscles resulted in return of full motility. High-dose corticosteroid treatment prevented further progression. The ptosis was complete and permanent, requiring a silicone frontalis sling procedure. (Contributed by Dr. Mark Duffy.)
