Subarachnoid hemorrhage (SAH), often occurring from rupture of an intracranial aneurysm, typically presents with a sudden severe headache and neck stiffness, and can be complicated by fatal rebleeding, arterial vasospasm producing ischemia, seizures, metabolic derangements, and venous thrombosis. The most common presenting complaint in SAH is the sudden onset of a painful headache followed by pain radiating into the occipital or cervical region. Meningismus develops as blood flows into the spinal subarachnoid space.

After carpal tunnel syndrome, the most common median nerve entrapment is the pronator teres syndrome. The most common features of the pronator teres syndrome are insidious proximal forearm fatigability, pain, and tenderness amplified by exercise and, at times, radiating to the shoulder. Other less common entrapment sites include the ligament of Struthers, lacertus fibrosus, and the tendinous origin of the flexor digitorum superficialis.

Cervical dystonia is the most common focal dystonia, consisting of involuntary head and neck movements, which result in abnormal postures, tremor, and associated pain. The most prominent feature is usually sustained rotational deviation of the head to one side. Neck pain is a common feature of cervical dystonia.

Myoclonus is an involuntary shock-like contraction of a muscle or a group of muscles, followed by relaxation. Myoclonic jerks are irregular in rhythm and amplitude and can range from mild muscular contractions with small amplitude movement to gross jerks affecting the whole body. Myoclonus occurs as a symptom or sign in numerous diseases and conditions, and the first step in focusing on the myoclonus is to determine its category of clinical classification.
