QTc prolongation and anteroseptal injury pattern in hypocalcemic myopathy secondary to idiopathic hypoparathyroidism

A 12-lead electrocardiogram of a 45-year-old woman showing a QTc interval of 486 ms (suggestive of borderline prolongation) and a pattern of acute anteroseptal injury. She presented in a wheelchair with complaints of generalized body aches and progressive weakness of both legs for 3 to 4 days. Chvostek’s and Trousseau’s signs were negative. Investigations revealed severe hypocalcemia (serum calcium 4.5 mg/dL) with inappropriately low-normal parathyroid hormone levels. ECG showed QTc prolongation and anteroseptal injury pattern.
