Although arsenic is more commonly known for intentional poisonings in homicides or suicides, the most common etiology of arsenic toxicity worldwide is ingestion of contaminated drinking water in endemic areas. The neuropathy related to arsenic is rarely seen without other systemic manifestations, including abdominal pain and skin changes. Arsenic neuropathy causes painful paresthesias and, with higher level or continued exposure, length-dependent weakness.

Lennox-Gastaut syndrome is an epileptic encephalopathy characterized by multiple seizure types, slow spike-wave pattern on EEG, and cognitive deterioration, beginning in infancy or childhood. Tonic seizures, atonic seizures, atypical absences, and myoclonic seizures may occur. West syndrome or various genetic conditions may precede the condition.

Epilepsy with generalized tonic-clonic seizures alone is probably a syndrome of idiopathic/genetic generalized epilepsy but of uncertain boundaries. Onset is usually in late adolescence, and genetic predisposition is common. Generalized onset tonic-clonic seizures may occur at any time, though those happening after awakening are more characteristic and better studied. Prognosis is markedly variable.

Vascular syndromes of the spinal cord are grouped into spinal cord infarction or transient ischemia syndromes, spinal cord hemorrhagic syndromes, and spinal vascular malformations. Pain is a disabling long-term complication in many patients, independent of motor function recovery. Complications include urinary tract infections or sepsis; pressure sores; autonomic dysreflexia; urinary, bowel, and sexual dysfunction; renal dysfunction; chronic pain; deep venous thrombosis or pulmonary embolism; respiratory dysfunction; and psychological or psychiatric manifestations.

Neurotrophic factors play an important role in the survival and maintenance of neurons. There are considerable experimental data on various neurotrophic factors in relation to peripheral neuropathies, which forms the basis of therapeutic interventions. There is some evidence of efficacy of neurotrophic factors in diabetic neuropathy in clinical trials and in anticancer agent-induced peripheral neuropathy, but no further clinical trials in are progress.
