Modafinil is indicated for the management of excessive daytime sleepiness associated with narcolepsy. The FDA has approved its use to improve wakefulness in patients with excessive sleepiness associated with obstructive sleep apnea/hypopnea syndrome and shift-work sleep disorder. Although modafinil improves measures of sleepiness, it does not normalize them because it does not correct the basic underlying cause. Therefore, treatment may be required for an indefinite period.

Pain is generally defined as an unpleasant sensory and emotional experience associated with actual or potential tissue damage or described in terms of such damage. Pain can be acute or chronic and is an important symptom of neurologic disorders. Management of chronic pain, particularly neuropathic pain, is a challenge. An understanding of the pathomechanisms of various types of pain is helpful in developing a management strategy.

Advances in the molecular biology of genetic disorders and the identification of gene mutations have enabled molecular diagnosis of these disorders. Molecular diagnostics are important for screening and as adjuncts to the clinical diagnosis of neurogenetic disorders. Next generation sequencing will improve the management and gene therapy of neurogenetic disorders in the era of personalized medicine.
