Tremor, the most common of all the movement disorders, is defined as an involuntary, rhythmic oscillation of a functional body region produced by alternating or synchronous contractions of reciprocally innervated agonist and antagonist muscles. Two subtypes of essential tremor pathology have been identified: a primarily cerebellar variant with Bergmann gliosis and Purkinje cell torpedoes, and a Lewy body variant.

Parkinsonism is a common side effect of all the atypicals except quetiapine and clozapine, certain calcium channel blockers, tetrabenazine, and its derivative vesicular monoamine transporter type 2 (VMAT2) blockers. It may also be seen with valproic acid and lithium. Although most drugs causing parkinsonism do so in a dose-related manner, there is an enormous variation in individual susceptibility.
