Acute decompensation of parkinsonism

This video demonstrates a man with Parkinson disease experiencing an acute worsening of parkinsonism over a few days, associated with a urinary tract infection. Examination reveals an almost constant 4-Hz rest tremor of bilateral hands and a 4-Hz jaw tremor. His gait is characterized by markedly short stride length and significant difficulty making steps during walking and turning, as well as a stooped posture. This represents a clear deterioration from his baseline (not shown), despite taking his usual dose of levodopa. (Contributed by Dr. Pichet Termsarasab.)
