Axial T1 fat-saturated MRI post-contrast in a 62-year-old man with Tolosa-Hunt syndrome revealed asymmetrical increase in enhancement and bulk of the right cavernous sinus extending laterally to the superior orbital fissure and orbital apex. The optic nerve, chiasm, and optic tract are normal in signal intensity with no focal enhancement. Both the orbital globe and ocular and extraocular muscles are unremarkable.
